When You Call or Visit

You can expect your first visit to Desert Streams to take approximately one hour. During this time, a therapist can begin to listen to your story and see how we can help you. If possible, please bring with you a completed Initial Assessment Form, Assignment and Release Form, and Insurance Form. All of these are available on our web site below.

If you are not sure about your specific insurance benefits, deductible or co-pay, please contact your insurance provider prior to your first appointment to answer any questions you may have regarding payment of services.

Desert Streams will accept most major insurance carriers, and we will file your insurance claims for you. Reduced fees may be available for those who do not have insurance benefits.

For your convenience, we accept Visa and MasterCard and offer the flexibility of scheduling evening and Saturday appointments.

Our Support Staff

Jackie Koop, Office Manager

Jackie joined the staff of Desert Streams in the fall of 2001. As part of the Desert Streams staff, she is responsible for client intakes and general office operations. She is married and has three children, and two grandchildren. Jackie enjoys reading, sewing, spending time with her family, and participating in activities at her church.
