When You Call or Visit

You can expect your first visit to Desert Streams to take appoximately one hour during which we begin to get your story and see how we can help you. If possible, please bring with you a completed Initial Assessment Form, Payment Responsibility Agreement, and Insurance Form. All of these are available on our web site below. If you are not sure about your insurance benefits, deductible or co-pay, we recommend you contact your insurance provider prior to your first appointment to get all your questions answered.

Desert Streams will accept most major insurance carriers and we will file your insurance claims for you. Reduced fees may be available for those not using insurance. We acept Visa and MasterCard and offer evening and Saturday appointments to fit busy family schedules.

Our Support Staff

Deb LaRoy, Office Manager

Deb was part of the original "dream team" who opened Desert Streams in 1991. She is primarily responsible for insurance billing and patient accounts. She and her husband Don have been married for 33 years, and they have one married daughter, Becky. Becky and her husband Greg are in their 3rd year at Western Theological Seminary and they reside in Holland. Deb enjoys riding motorcycle with her husband, walking the pier in South Haven, antique hunting, reading, and spending time with family and friends. She is a leader for Children & Worship at Twin Lakes Church and of an adult small group Bible Study.






Jackie Koop, Administrative Assistant

Jackie joined the staff of Desert Streams in the fall of 2001. As part of the front office staff she is responsible for intakes, data entry, and assisting with general office operations. She and her husband Clay have been married for 34 years. They have one daughter, two sons, and one delightful grandson, Silas. Jackie enjoys reading, sewing, and spending time with family. She attends Galesburg Baptist Church.


Gay Gilbert, Administrative Assistant

She is affectionately known as "Grandma Gay" at her church where she is active in the children's ministry. She has 3 adult children and 6 grandchildren. She likes to crochet and sew in her spare time. She praises God for bringing her to Desert Streams where she uses her education and experience in the clerical field.
